Complete only the forms that apply to your situation. If the Financial Aid Office has asked you for documentation, follow its instructions about which form to use and how to submit it.

2026–27 FAFSA verification forms

If º£½ÇÉçÇø asks you to verify information from your FAFSA, use the form or forms specified by the Financial Aid Office. You may not need every document listed here.

• Asset Information Worksheet — Provide additional asset information if requested.

• Identity and Statement of Educational Purpose (college) - Use when the Financial Aid Office directs you to verify your identity through the college.

• Identity and Statement of Educational Purpose (notary) - Use the notary version if the Financial Aid Office instructs you to do so.

• Proof of dependent support - Document support provided to a dependent if requested.

• Verification worksheet - Confirm FAFSA information identified for verification.

Appeals and other aid requests

If your circumstances have changed or you need to request aid for a specific term, start with the relevant form below. Contact Financial Aid if you’re unsure whether an appeal is appropriate for your situation.

Student and parent loans

Review the applicable instructions if you’re considering a federal or private education loan. You do not need to complete a loan form unless you choose to pursue that option or the Financial Aid Office directs you to it.
